In this article we will walk you through the process of creating a care plan and managing scheduled care tasks.
To access this feature, open the patient’s record and select the Care Plan option. From there, you can set up the patient’s care plan and schedule recurring events. To add a specific care plan task, click Add Task.
When the form opens, begin by selecting the Activity from the dropdown list and specifying the required Level of Care.
Next, choose the Frequency for the task. Once the frequency is set, the system will prompt you to define specific time slots within the day by selecting the hour and minute for each occurrence.
If a task is scheduled to occur more than once a day, you will be prompted to define specific times for each occurrence. For example, if you select Three Times Daily, you will need to set the time for each instance by choosing the hour and minute for each occurrence.
If you select the task frequency as Weekly, in addition to setting the hour and minute for each occurrence, you will also need to specify the day of the week.
Similarly, for a Monthly frequency, you will need to define the specific day of the month on which the task should occur
Next, add the Time Required by entering the number of minutes needed to complete the task.
Once the task details have been configured, assign the appropriate groups to the task. It is recommended to include both Carer and Nurse groups.
Please note that only the groups assigned to the task will be able to view and update these care tasks from the Facility Rounds screen.
A General Notes section is available for adding any additional details about the task
Once you are satisfied with the information entered, click Add to List on the left side of the screen. The task will then appear below in the modal.
You can add multiple care tasks by repeating the steps above. Once the care plan is complete, scroll down and click Save
If a care task is no longer valid, you can deactivate it by toggling the Activate slider to deactivate. Please note that once a care task has been deactivated, it cannot be reactivated.
To make changes to an existing care task, the task owner can edit it directly from this screen by clicking on the field that needs to be updated
Once completed, navigate to the Clinical Instructions section in the patient record and select the Care Tasks from the dropdown options.
This view displays all care tasks for the patient, with each scheduled occurrence listed individually. For example, if a task is scheduled daily, you may see separate entries for today and yesterday.
Each task instance can have its status updated individually to reflect the appropriate outcome. You can also add specific comments for each task by clicking on the blue hyperlinked activity name.
You can also switch the view from List to a Timeline, where scheduled tasks are displayed as blue bubbles at their assigned times across a 24-hour period.
Use the navigation buttons above the timeline to move backward or forward between days.
Clicking on a bubble will open a list of all tasks scheduled for that time, where you can add notes and update the status of each task.
If the care task activity is set to a meal - such as Breakfast, Lunch, Snack, or Dinner - instead of selecting Care Tasks under the Clinical Instructions screen you will access it through Kitchen Rounds
Alternatively, instead of navigating between multiple patient profiles to manage and track care tasks, you can select Facility Rounds from the left-hand menu and click on Care Rounds from the horizontal options. This provides a consolidated view of all patients in the facility, along with their scheduled tasks and corresponding times.
The colored circles indicate the status of each scheduled task: blue for tasks still to be completed, orange for partially completed tasks, red for missed tasks, and green for completed tasks.
