In this article we will walk you through how to input and edit clinical scores, specifically FIM + FAM scores.
This feature was originally designed using the US FIM + FAM model, but it now been fully redesigned to align the UK FIM + FAM structure.
Important to Note:
Patients who already have clinical scoring initiated under the original US version will continue using that version to maintain consistency in their records and reporting. Only new patients—those without any existing FIM + FAM scores—will automatically use the updated UK version moving forward.
To access clinical scores, navigate to the patient file and select Clinical Assessment on the horizontal menu. Then navigate to Clinical Scores on the far right.
Similar to recording Vital Signs, the top horizontal tick box menu is there for you to decide which clinical scores you would like to view. You access this by clicking on the green eye.
To capture a FIM + FAM score, click on the + icon and select FIM + FAM from the screen that opens.
FIM + FAM scores are divided into three sections:
1. Admission
2. Current
3. Goals
If you have grown accustomed to using the US FIM+ FAM model, then it is important to note that the order of these sections have changed, and some of the scoring tasks may differ slightly.
The Admission and Goals scores are set only once, so are locked by default, to edit them select the check box on the top right corner of the section you would like to complete, after you finish entering the scores, be sure to clear the checkbox to lock them in.
Important to note:
You cannot record an Admission score and a Current score in the same entry. If you attempt to capture both, the second value entered will overwrite the first. For example, if you enter an Admission score and then move to the Current score column, a warning will appear indicating that saving the Current score will replace (and remove) the Admission score.
Hovering over the (i) icon at the end of each item will display a sample score scale relevant to that particular task to aid in maintaining scoring consistency.
When entering new FIM+FAM scores, each field will display a placeholder showing the previously recorded score for that task.
Remember to set all scores across all the categories shown horizontally. When you’re finished, click Save.
When recording a patient’s discharge scores, you can select the Discharge Record button located at the bottom left of the screen. This will mark the entry as a Discharge record.
Additionally, if a patient’s Arrival Status is changed to Discharged, the most recently recorded Current score will automatically be updated and labelled as the Discharge score.
Expanding the FIM + FAM carousel menu will display these scores. On the left, in green we have the goal score and in yellow we have the score that was taken on admission. Then, right-to-left we have the scores over time.
Additionally, each category will have its own subtotal, which is calculated based on the category score scale and the scores recorded.
Clinical scores can also be accessed under the Rehabilitation Plan screen
When generating reports—such as the Authorisation Report or the Specialist Discharge Report—you can choose whether to include the patient’s FIM+FAM scores or alternatively, select the FIM-only option, which will exclude the FAM scores from the report.
FIM + FAM records are now available on the Facility Rounds screen, just select the FIM + FAM option from the horizontal menu.
This means you can view scores for all patients at a glance without needing to open each individual patient record.
The sections highlighted in pink indicate where each patient’s FIM + FAM scores are incomplete.
The Facility Rounds screen includes dynamic filtering that lets you tailor exactly what information you want to see. You can quickly switch between viewing FIM only or FIM+FAM, and you can choose which types of entries appear—for example, Admission scores, Current scores, Goals, or Discharge information—either individually or in any combination you need. The screen can also be adjusted to display the Admission and Discharge dates, or to show the captured scores instead of the totals. Additionally, you can select which specific categories you want displayed, allowing you to focus on the areas most relevant to your review
Also displayed on his screen are the Efficiency Score and the Achievement Score. The Efficiency Score reflects the rate of a patient’s functional improvement over the course of their rehabilitation stay, while the Achievement Score represents the total change in the patient’s functional status from admission to discharge
