In this article we are going to cover a new module created, Incident reports also called Adverse Events and all its features and applications.
To create an Incident Report, click on the tick icon at the bottom right of your screen, this will appear in all the dashboards and inside the patient record
A tasks panel will open on your right, default will be tasks, to change it, select Incident Report on the top right.
You are the also able to select whether it is a Clinical or Non-Clinical Incident
Complete the top section of the form
Incident Type
Priority
Due Date & Time
Patient (This will be automatically populated if inside a patient record, if on one of the dashboards the patient would need to be selected)
Facility will = the current one you are in
Status default on creation of Incident Report will be In Progress
Once the above has been completed the Start Report Form button will turn blue, you can select the SAC Score (Severity Assessment Score) not a compulsory field

Then click Start Report Form
The form to complete details of what occurred during an incident will open up, there are 8 steps or screens to the form for Clinical Incidents.
In Step 1 the circled information is compulsory
When the appropriate fields are completed, click Next
Step 2 has no compulsory fields, once appropriate fields have been completed, click Next
Step 3 also has no compulsory fields, when appropriate fields have been completed, click Next
Step 4 also has no compulsory fields, you are able to select a record of vital signs to be stored in the incident report, when appropriate fields have been completed, click Next
Step 5 the circled information is compulsory, when completed, click Next
Step 6 Incident Reported By is compulsory, it is a free text field, we made provision for the situation that if a non HealthWare staff member like a cleaner or a external provider is the individual who came across the incident, that their name could be added as the first person on the scene. In this case the most senior staff member will take their account of what transpired.
Step 7 no compulsory fields, click next if/when completed
Step 8 also no compulsory fields, if any witnesses, enter their details click Add+, it will the allow you to add additional witnesses if more than one was present.
Click Save Changes
If there were any fields missed that were compulsory as part of the initial completion of the form, HealthWare will prompt you and advise you what was missed.
You can then click Previous until you get to the Step missed, complete whats needed, then click, Save Changes
The form will be Orange in colour, meaning that it's still being completed to it's full extent ie In Progress
To save click, Create and Close Incident Report
The incident will save in the patient record under Clinical Instructions, select dropdown Incident Reports
While the Form is in an In Progress Status (which means it's editable), only the user who has created the form can access the form details unless the user checks the Public Record box.
In the below example, I'm logged in as Zeenat, but am trying to view a form created by Admin2, I am able to see a form exists but not able to access any information yet as the form is still In Progress and Admin2 has not marked it for public viewing.
To edit or view the details of the incident click on Incident Type, the panel will open to the right.
Any video links or attachments will save in the chats/comments section at the bottom of the report
To edit or view the details, click View Report Form
The form will open back up and additional information can then be added, once the incident report is completely finalised and all details required have been added, Sign Off Report Form As Completed, Save Changes.
Once Incident Complete has been selected, the incident details will be locked and the user will no longer be able to edit any information inside the form, but will be allowed to add Comments in the task if needed.
As the incident details are now complete, other users will also be able to access the form to view the details of the Incident.
Once the incident has been reviewed the Status can be changed to Requires Investigating
The User (Orange Arrow) or Group (Purple Arrow) who is required to do the investigation can be tagged in the comments panel, they will the receive a notification.
The comments panel in Incident Reports, works the same as Tasks, and is date and time stamped, including when statuses of the form are changed.
Once the person starts, the status should be changed to Investigating.
That will open up the part of the form for the Investigation information.
Select Investigation and complete the 4 steps of the form, Save Changes.
The Investigation part of the form will remain open for edits until Investigation Complete is selected. This will also lock the Investigation Form details.
A record of all the Incident Reports per facility will be stored in the Facility Reporting - Incident Reports Screen
Two weeks worth of data will be visable as a default, you are able to change this by click on the dropdown and selcting, 4 weeks, 2 months, 3 months, etc or a custom date range.
The data can then be exported to CSV for extraction.
